Complete this form to consent to receiving SMS notifications from Preventa Wellness, including report availability alerts, appointment reminders, and account security codes.
Preventa Wellness, LLC ("Preventa," "we," "us," or "our") is a wellness intelligence platform that analyzes lifestyle and behavioral data to generate personalized wellness insights. Preventa is not a medical product or medical service and does not claim to treat, cure, diagnose, or prevent any medical condition.
Our SMS/text messaging program sends notifications related to your wellness assessments, including upload confirmations, report availability alerts, appointment reminders, and account security codes. Messages are sent from a single United States phone number.
What messages
you may receive.
Upload Confirmation
A notification sent after your wellness assessment data has been received and processing has begun.
Report Ready
An alert sent when your personalized Preventa Wellness report is ready to view on our secure client portal.
Appointment Reminder
A reminder sent before an upcoming wellness assessment session at an authorized partner location.
Two-Factor Authentication
A one-time verification code sent during account login for security purposes.
You may receive up to 6 messages per month depending on your assessment activity and account usage.
Message and data rates may apply. Preventa Wellness does not charge for SMS messages, but your mobile carrier's standard messaging rates apply. Contact your carrier for details about your messaging plan.
How consent
is collected.
You complete the consent form above with your name, phone number, and explicit opt-in checkbox — either online or at an authorized Preventa Wellness partner location during in-person enrollment.
Your consent is recorded in our system with a timestamp.
For two-factor authentication (2FA) messages, consent is implicit when you initiate a login and request a verification code.
Consent to receive SMS messages is not required as a condition of purchasing any goods or services from Preventa Wellness.
How to
opt out.
Reply STOP to any message you receive from Preventa Wellness to immediately opt out of all SMS messages.
You may also opt out by emailing support@preventawellness.com or calling (602) 932-2284.
After opting out, you will receive a one-time confirmation message and will no longer receive text messages from Preventa Wellness unless you opt in again.
To re-subscribe, reply START to any previous Preventa Wellness message.
